Job Offer for the position: Coordinator of the Environmental Self-Help Home type AC for adults with chronic mental illnesses and individuals with other chronic mental functioning disorders

Expectations:

  • higher education, preferred humanistic fields, e.g. pedagogy, sociology, etc., applicable in the provision of services in the Home, including a specialization in social assistance organization;

  • at least six months of professional experience in providing services for
    individuals with mental disorders in the areas of: skills to shape motivation for socially accepted behaviors, developing habits of purposeful activity, conducting social skills training;

  • experience in preparing individual supportive-activating action plans and assessing their results

  • experience in conducting social skills training;

  • knowledge of computer operation and MS Office programs;

  • independence, good organization of work, ability to work in a team;

  • communication skills, engagement, creativity, high personal culture, empathy;

  • ability to organize various types of educational-promotional events, in collaboration with local media and other institutions providing social and cultural-service offerings.;

Preferred knowledge of:

  • The Act of March 12, 2007 on Social Assistance with subsequent amendments (Dz. U. of 2021, item No. 2268 Announcement of the Marshal of the Sejm of the Republic of Poland of November 17, 2021 on the announcement of a uniform text of the Act on social assistance);

  • The Act of August 19, 2005 on counteracting violence with subsequent amendments (Dz. U. 2021 item 1249 Announcement of the Marshal of the Sejm of the Republic of Poland of June 16, 2021 on the announcement of a uniform text of the Act on counteracting domestic violence);

  • The Act of March 8, 1990 on municipal self-government with subsequent amendments (Dz. U. of 2023 item 40 Announcement of the Marshal of the Sejm of the Republic of Poland of November 4, 2022 on the uniform text of the Act on municipal self-government);

  • The Act of May 24, 2018 on the protection of personal data with subsequent amendments (Dz. U. 2023, item 1206);

  • Regulation of the Minister of Labor and Social Policy regarding environmental self-help homes of December 9, 2010 (Dz. U. of 2010 No. 238, item 1586) with subsequent amendments.

The duties of the employed person will include:


Administrative– office organization of the facility, including documentation related to the participant's situation and activity logs.

  • developing the activity program and work plans;

  • inspiring, coordinating, and implementing educational and methodological work among participants aimed at improving activity, proper psycho-motor condition, prosocial behavior, and hygiene, as well as safe behavior;


Developing the annual and daily work plan of the Home and its implementation;


Organizing high-quality activities and care standards;

  • Preparing statistical and descriptive reports on the Home's activities;

  • Preparing individual supportive-activating action plans for participants of the facility and assessing their results;

  • Working in a supportive-activating team;

  • Co-organizing various types of training for participants of the Day Support Center, including:

– functioning in everyday life (e.g., taking care of personal appearance, learning hygiene, culinary training, practical skills, managing personal finances),

– interpersonal skills and problem-solving (e.g., fostering positive relationships between the participant and close individuals, neighbors, other people during shopping, in public transportation, in offices, in cultural institutions),

– skills in spending leisure time (e.g., developing interests, participating in social and cultural events)

  • providing necessary care for participants of the Day Support Center, including assistance with official matters and health services

  • preparing necessary documentation specified in the Regulation of MPiPS of December 09, 2010 on environmental self-help homes

  • cooperation with institutions and the local community to ensure the social integration of participants of the Day Support Center.
