How and when to collect the ID card and badgeHonorary Blood Donor? Check the answers to the most frequently asked questions.

I submitted an application for the Honorary Blood Donor Badge and what next?

All applications from RCKiK from Mazovia are sent to RCKiK Warsaw and from there to MOO PCK. After a month from submitting the application, contact us via email: warszawa.zhdk@pck.pl. We will check if the application has reached us and then it will be possible to come to us.



What to do if I donated blood at different centers in Poland?

All certificates received from other blood donation stations (MON, MSWiA, RCKiK Radom, other provinces) should be submitted to the Blood Donation and Hemotherapy Center, the one where you most often donate blood – preferably at RCKiK Saska because it maintains the database for the entire Mazovian province. RCKiK will enter all data into its database and will print the application for the ZHDK badge based on that. When the application is delivered to us, we will send the information via SMS with a link to this page.



Is my ID card waiting for me?

The ID card is printed on-site on the day the interested party arrives (Tuesdays and Thursdays). If you submitted an application at RCKiK, contact MOO PCK after a month from the submission of the application – email: warszawa.zhdk@pck.pl. If you received a certificate of the amount of blood donated from RCKiK Radom or an application for ZHDK directly from the station, please bring it to us on the specified days. If you submitted an application at a station outside of Warsaw on a blood bus outside Warsaw, the ready ID cards will go to the nearest PCK District Branch OR in Mazovia and will be available for collection there.



On what days can I pick up the ID card?

You can collect the ID card EXCLUSIVELY on Tuesdays and Thursdays from 08:15 to 15:30.



Where can I collect the ID card?

We accept at the Mazovian Regional PCK Branch at Szańcowa Street 25, in Warsaw.


Can I come by car?

We do not recommend this solution. There are no available parking spaces near the Center. If you cannot come by public transport, leave your car in the parking lot at the Wola Park shopping center.



How to get to you?

By metro – Ulrychów station (exit to the opposite side of the shopping center) or by bus at the Metro Ulrychów stop.


I have a paper ID card. What should I do with it?

If you have a paper Honorary Blood Donor ID card, please bring it with you. You will hand it over during the issuance of the ZHDK ID card.



How to obtain a duplicate of the Honorary Blood Donor ID card?

Applications for a duplicate of the ID card are submitted at the PCK branch where you received the original. If it was at the Mazovian Regional PCK Branch, write to: warszawa.zhdk@pck.pl. When filling out the application for issuing a duplicate, ensure that your signature is on all documents, including the GDPR consent.

Important: make sure that all data on the documents is correct, preferably IN PRINTED LETTERS. Any errors will result in the ID card being issued incorrectly.




Can I receive the ID card by post?

Individuals living more than 30 kilometers from Warsaw can receive the ID cards by post – registered mail. Please contact us at warszawa.zhdk@pck.pl



Can someone else pick up the ID card for me?

Write to us at warszawa.zhdk@pck.pl we will send a template of the authorization for collection to be filled out and provided to the person collecting the ID card.




How can I check how much blood I donated?

To determine how much blood we have donated, we can obtain a certificate confirming the amount of whole blood and its components donated in a given calendar year from RCKiK or the local branch. Starting from 2024, with the introduction of the pilot version of the e-blood system, such a certificate can be obtained through the Internet Patient Account.
